Building Substitute Request Form

Please complete the below form for building substitute requests. All requests should be reported to TOC as soon as possible. It can take 1-3 weeks to recruit, onboard, and place a qualified candidate. After completing this form, TOC will begin contacting candidates and send the names of those selected to the contact(s) noted below. At that time, we will also confirm how the building substitutes should be scheduled in the absence management system.
